Home Care Package(HCP)

 

Hills Physio Care and Allied Health provides a variety of services for older clients who have a Home Care Package.

What is Home Care Package?

A Home Care Package is a government-subsidised care program that provides access to coordinated services to help senior Australians live independently in their homes for longer.
Once you are assessed and allocated a Home Care Package,you receive an unique referral code that allows you to choose a Home Care Package provider to deliver the services you require.

Our expertise in neurological conditions, including dementia, as people become older can assist you and your family in obtaining helpful advice and put in place the appropriate strategies to maintain wellness, reduce falls and maximising independence and physical ability for as long as possible.
